Security of the Patient and Medication Reconciliation

It is a training activity aimed at improvement of the patient safety in general and to the improvement of the safe and proper use of medications in particular. Specially the reconciliation of medication in the hospital care, primary care interface and conciliation to the hospital discharge. It is a activity where the basic security contents are exposed, and a practical part where theoretical knowledge will develop the basic tools for the identification and analysis of risks and the effective implementation of the reconciliation of medications.

Training aims

General objectives

  • Acquire basic knowledge about patient safety and learn the essential tools for the identification and analysis of risks that will serve as the basis for an effective implementation of a program of reconciliation of medications.

Specific objectives

  • Manage fundamental concepts of the patient safety
  • Know magnitude and significance of the safety of the patient and his main studies.
  • Handle tools necessary for developing a risk map.
  • Know main tools of risk analysis and identification of causes.
  • Applying knowledge acquired with specific examples.
  • Know types of errors of medication and principal causes according to chain drug and Therapeutics.
  • Know main safe practices in the use of medications.
  • Know basics and methodology of the reconciliation of medications.
  • Present basic activities of the pharmacist in dealing with a program of reconciliation of medications.
  • Handle main recommendations in security by groups of drugs.
